Welcome to DFW-IV (www.dfw-iv.com). These Terms and Conditions ("Terms") govern your use of our website and the purchase of our mobile IV hydration and wellness services. By booking an appointment or using this website, you agree to be bound by these Terms.

1. MEDICAL DISCLAIMER

The services provided by DFW-IV are intended for hydration, vitamin replenishment, and general wellness.

  • Not a Substitute for Medical Care: The information on www.dfw-iv.com and the services provided are not intended to diagnose, treat, cure, or prevent any disease.

  • Professional Consultation: You should consult with your primary care physician before beginning any new treatment. In the event of a medical emergency, call 911 immediately.

  • FDA Disclosure: Statements regarding vitamin therapy have not been evaluated by the Food and Drug Administration.

2. ELIGIBILITY AND CONSENT

  • Age: You must be at least 18 years of age to book a service. Individuals under 18 must have a legal guardian present to provide written consent.

  • Health History: You agree to provide a complete and accurate health history. DFW-IV is not liable for complications resulting from undisclosed health conditions, allergies, or medications.

  • Medical Assessment: Per Texas law, all IV services require an order from a licensed medical prescriber. You consent to a "Good Faith Exam" via telehealth or in-person prior to your first treatment.

3. APPOINTMENTS AND CANCELLATIONS

  • Booking: Appointments are subject to the availability of our mobile nursing staff.

  • Cancellation Policy: We require at least 24 hours' notice for cancellations. Cancellations made with less than 24 hours' notice, or "no-shows" at the time of the nurse's arrival, will be charged a $50.00 cancellation fee.

  • Right to Refuse: Our medical staff reserves the right to refuse service if a patient’s vital signs are outside of safe operating limits or if the environment is deemed unsafe

4. PAYMENT AND MEMBERSHIPS

  • Payment: Payment is due in full at the time of service. We do not accept health insurance.

  • Subscription Terms: If you enroll in a DFW-IV Membership, you authorize recurring monthly charges to your provided payment method. Memberships may be canceled with 30 days' notice. Benefits are non-transferable.

5. LIMITATION OF LIABILITY

To the fullest extent permitted by Texas law, DFW-IV, its owners, and its staff shall not be liable for any indirect, incidental, or consequential damages. Our total liability for any claim arising out of our services shall not exceed the amount paid for that specific service.

6. GOVERNING LAW AND JURISDICTION

These Terms and Conditions, and any dispute or claim arising out of or in connection with them or their subject matter (including non-contractual disputes or claims), shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions. You agree that any legal action or proceeding between DFW-IV and you shall be brought exclusively in a court of competent jurisdiction sitting within the State of Texas.

7. ELECTRONIC COMMUNICATIONS (SMS & EMAIL)

By providing your phone number and email address to DFW-IV, you expressly consent to receive electronic communications from us.

  • Service Notifications: You agree to receive automated SMS and email messages related to your appointments, such as booking confirmations, nurse arrival updates, and post-treatment follow-ups.

  • Marketing & Promotions: If you opt-in, we may send promotional messages regarding new treatments or membership discounts. You are not required to opt-in to marketing as a condition of receiving services.

  • Opt-Out (SMS): You may opt-out of text messages at any time by replying "STOP". You will receive one final text confirming your unsubscribed status.

  • Opt-Out (Email): You may unsubscribe from marketing emails by clicking the "Unsubscribe" link at the bottom of any message.

  • Message & Data Rates: Standard message and data rates from your mobile carrier may apply. DFW-IV is not responsible for any fees charged by your provider.

  • Privacy of Health Information: While we use secure systems, SMS is not a fully encrypted medium. By consenting to SMS, you acknowledge the inherent risks of transmitting health-related appointment reminders via text.
